Program Overview

For Transitional Age Youth (Ages 18–25)

Leading By Example’s Residential Rehabilitation Program (RRP) is designed to support Transitional Age Youth (TAY: young adults ages 18 through 25) as they navigate the critical transition from adolescence to independent adulthood. This program offers safe, stable housing alongside comprehensive behavioral health and life skills support.

Our goal is to empower youth to live independently in the community while receiving the individualized care and structure they need to succeed.

What Our Program Includes

Leading By Example’s Residential Rehabilitation Program (RRP) provides safe, supportive housing paired with personalized mental health services, life skills training, and educational and vocational support. Our comprehensive approach equips young adults with the tools they need to thrive independently and confidently in their communities.

Key areas of focus include:

  • Supportive housing
    Residents live in a safe, community-based setting that fosters autonomy, accountability, and personal growth, while still providing access to staff and services that promote mental wellness and recovery.
  • Clinical services
    All participants have access to:

    • Medication management
    • Individual and group therapy
    • Ongoing care coordination with mental health providers
  • Life skills and independence
    Youth are supported in developing the skills needed for independent living, including budgeting, cooking, time management, and self-care practices. The program also encourages responsible decision-making and goal-setting.

  • Vocational and educational support
    Through supported employment services, residents receive job readiness training, help with finding and maintaining employment, and assistance in accessing educational opportunities, including GED programs or college enrollment.
  • Community integration
    A major focus of the RRP is helping young adults become active and engaged members of their communities, whether through work, volunteering, school, or peer support networks.

Who Is Eligible

Youth (Ages 18-25)

  • Young adults aged 18–25 who have evidence they cannot live safely in the community (example: released from incarceration, violent act, homelessness, at risk of homelessness, couch surfing, eviction, etc.)
  • Diagnosed with a serious mental health condition
  • In need of structured support to transition into or remain in community living

Program Goals

  • Promote mental health stability
  • Foster independent living skills
  • Support educational and vocational success
  • Enhance self-confidence and self-sufficiency
  • Prevent hospitalization, homelessness, or incarceration

Why It Matters

The transition period between youth and adult for those with a mental illness adds an extra layer of challenge.

Transitional age youth are often underserved in the mental health system, despite facing some of the highest risks for housing instability, unemployment, and untreated mental health conditions. Our RRP provides a safe landing space and a springboard for independence, meeting young adults where they are, and walking alongside them toward where they want to go.

How to Make a Residential Rehabilitation Program Service Request Form

Leading By Example is now accepting referrals for our Transitional Age Youth (TAY) Residential Rehabilitation Program (RRP) located in Baltimore City.

To refer to our program:

  • Complete the State of Maryland RRP Referral Form

  • Submit the referral to the appropriate local behavioral health authority (LBHA) based on where the individual currently resides. Contact information for each LBHA can be found on page 3 of the referral form linked above.
  • To request placement specifically with Leading By Example’s Baltimore City TAY RRP: In Section L (“Additional Comments”) of the referral form, write: “Leading By Example.”
