Program Overview
For Children & Adolescents Ages 5–17 in Baltimore City
Our Youth Care Coordination services are designed to support children and adolescents (ages 5–17 at enrollment, once enrolled you are eligible for the services until age 21) in Baltimore City who are experiencing behavioral health challenges or have a psychiatric diagnosis. We partner closely with families to ensure youth and the family are connected to the care, trusted resources, and opportunities they need to thrive at home, in school, and in their communities.
Care Coordination helps families navigate the often overwhelming landscape of medical, mental health, educational, and social support systems by providing a central point of contact and consistent, compassionate guidance.
What Our Program Includes
Our services follow a holistic, wraparound model that is youth-guided and family-driven, meaning care plans are designed around the unique strengths, needs, and goals of each youth and family.

Minors (Ages 5–17)
- Must be aged 5–17 at enrollment (once enrolled you are eligible for the services until 21 years old)
- Must have demonstrated behavioral/emotional needs or have a behavioral health diagnosis

Navigating the behavioral health system can be overwhelming – when families are already under stress. Youth Care Coordination is designed to bridge gaps in care, break down systemic barriers, and ensure families have the consistent support they need.
With the help of a dedicated Care Coordinator, families don’t have to go it alone. We walk alongside youth and caregivers to create clear pathways toward wellness, growth, and lasting change.
