Youth Care Coordination

Care Coordination Services are provided to assist participants ages 5 to 18 with psychiatric diagnoses and families living in Baltimore City in gaining access to needed medical, mental health, social, educational, and other services. The primary goal is to ensure needs are met and participants are stabilized if need be, to support in connecting the client and family with the appropriate level of care needed.
