For Adults in Baltimore County

Leading By Example’s Targeted Case Management (TCM) program helps adults (18+) with behavioral health needs gain access to essential services and maintain stability in the community. TCM is designed to reduce the risk of hospitalization, homelessness, or incarceration by ensuring individuals receive the care and support they need, when they need it most.

Through personalized case management, participants are empowered to improve their quality of life, manage their mental health, and maintain their independence.

What Our Program Includes

TCM participants are assigned a case manager who provides individualized support based on their level of need.

Key areas of focus include:

  • Initial assessment and ongoing reassessment
    Every participant receives a comprehensive intake assessment and is reassessed every six months to adjust support based on current needs.

  • Collaborative care planning
    Development of a strengths-based care plan tailored to address medical, mental health, housing, education, and social service needs.
  • Service coordination and linkages
    Connecting individuals to:

    • Outpatient mental health or substance use services
    • Primary care and specialist providers
    • Housing and food resources
    • Vocational and educational supports
    • Transportation and entitlements
  • Advocacy and follow-up
    Case managers advocate for participants in healthcare, housing, and legal systems, and follow up to ensure services are accessed and maintained.

Who Is Eligible

Adults (Ages 18+)

  • Be 18 years or older
  • Have a mental health diagnosis from a previous or current mental health provider
  • Reside in Baltimore County

Program Goals

  • Prevent unnecessary hospitalizations or legal involvement
  • Ensure access to and coordination of needed services
  • Support mental health stability in community-based settings
  • Enhance participants’ ability to live independently
  • Promote long-term wellness and self-sufficiency
  • Connections to services, such as:

    • Behavioral health treatment (therapy, psychiatry)
    • Advocacy
    • Housing, food, and transportation resources
    • Employment or Vocational programs
    • Entitlement support (e.g., Medicaid, SSI, SNAP, housing vouchers, etc.)

Why It Matters

Individuals with a mental illness often struggle with finding the best services for them.

Many individuals with mental health conditions struggle to access the right services at the right time. Without coordination, care can become fragmented, leading to crises, emergency room visits, or loss of housing or employment. TCM bridges the gap by offering consistent, person-centered support that helps individuals remain stable, supported, and connected to the resources they need to thrive.
